ELC Attendance Notification Form

Parents,

Please use this form to notify the ELC about changes to your child's attendance. Complete a separate form for each child.

Please complete this form no later than 9:00 am on the day your child will be arriving late, departing early, or absent for some or all of the selected school day. If you are notifying us about a vacation, please indicate whether or not you would like to use one or both of your two tuition-free vacation weeks. We ask for a month's notice to process vacation week requests.

If your child is absent due to illness for three or more days, Fay School requires a note from your child's physician to return to school. Please contact the Health Office.

Families enrolled in the Early Learning Center are entitled to two weeks of tuition-free vacation each year. The vacation should be requested at least one month in advance, and the absences must be in 5-consecutive school day periods (i.e., Monday-Friday or Wednesday-Tuesday). The two weeks may not be used during the ELC Summer Sessions. Please review the ELC Parent Handbook for more information.

ELC is closed from December 23-27, 2024.
